Background
The Community Health Management (CHM) Task Force, composed of 13 National Training and Technical Assistance Partner (NTTAP) organizations, was developed to build off of previous work and lessons learned 2017 - 2024 of the Task Force. The purpose of the CHM Task Force is to engage health centers, Primary Care Associations (PCAs), and Health Center Controlled Networks (HCCNs) to address critical issues to improve the prevention, treatment, and management of chronic conditions among at-risk patients in health centers nationally.
