Diabetes in Special and Vulnerable Populations:
A National Learning Series

Background
Diabetes affects more than 34 million people in the United States. Multi-tiered efforts to prevent, treat and manage diabetes are critical in reducing the burden of diabetes, particularly for special and vulnerable populations, which have unique characteristics that affect culturally and linguistically competent healthcare access and utilization. According to the 2018 Uniform Data System (UDS), diabetes poses a unique challenge for the HRSA Health Center Program because 1 in 7 patients has diabetes and nearly 1 in 3 of those has uncontrolled diabetes.
To elevate the national conversation around diabetes, 14 National Training and Technical Assistance Partner (NTTAP) organizations formed the Special and Vulnerable Populations - Diabetes Task Force to engage health centers, Primary Care Associations (PCAs), and Health Center Controlled Networks (HCCNs) to increase knowledge of effective strategies that address diabetes among people experiencing homelessness, residents of public housing, migratory and seasonal agricultural workers, school-aged children, older adults, Asian Americans, Native Hawaiians, and Pacific Islanders, LGBTQIA+ people, and other health center patients.
This Fall’s national learning series will take a deeper dive into issues around systemic, structural, and environmental barriers and strategies for improving diabetes care. This series complements HRSA’s Diabetes Quality Improvement Initiative: click here.
Purpose
Each webinar will focus on specific strategies for developing, evaluating, and supporting effective health care delivery models designed to meet the specific needs of special and vulnerable populations. Coordinators will ensure that webinar materials are practical, providing information and enhancing skills directly related to the training recipients. Based on HRSA recommendations, future conversations will focus on health centers and incorporate how NTTAPs, PCAs, and HCCNs support diabetes quality improvement around various topic areas (i.e. food security, clinical guidelines, reimbursement, technology utilization, etc.)
An application for AAFP for continuing education will be submitted for the webinar series, “Diabetes in Special and Vulnerable Populations: A National Series.” The total number of CEU is tentative and will be determined once AAFP reviews the application. As per AAFP regulations, all participants seeking continuing education must be present for all four sessions and submit the post-evaluation for each. Check this page often for updates on CEU credits.

Webinar #1: Diabetes Continuum of Care: Understanding Social Determinants of Health and Structural Barriers Affecting Diabetes Care

Webinar #2: Diabetes Continuum of Care: Intersectional Identities of Health Center Workforce and Patients

Discuss the impact of provider burnout on diabetes care and explore how to allow more agency and autonomy throughout the care team;
Discuss intersectional identities and staff reflecting patient populations and how this relates to staff retention;
Explore tools, resources, and assistance in addressing provider burnout, moral injury, and stress in order to improve diabetes management for patients.

Webinar #3: Diabetes Continuum of Care: Emergency Preparedness for Diabetes Management

Summarize the role of environmental justice issues on diabetes management and access to care for special and vulnerable populations;
