Diabetes Continuum of Care - June 2023

These publications include a collection of key resources, lessons learned, and effective strategies identified during our March 2023 Learning Collaboratives to address diabetes among special and vulnerable populations along the HRSA-defined diabetes continuum of care. A total of four (4) new publications were created by the Special & Vulnerable Diabetes Task Force.
